Inquiries and professional referrals

When you submit a form, the contact details and information you enter are sent to the Willow Oak team so we can respond to your inquiry or referral. Web3Forms processes delivery, and Cloudflare hosts the website and form endpoint. Potential resident contact details are optional. Please provide another person’s details only with permission to make the introduction.

These forms are not secure medical-record intake and are not for emergencies. Do not submit medical records, diagnoses, financial information or other sensitive details. A referral is not a reservation or admission approval.

Website features

The embedded map loads from Google, which receives normal connection information such as your IP address. Automatic invitation dismissals are stored in your browser’s tab session, so they stay dismissed on refresh. The site does not save unfinished form entries to persistent browser storage.

Website analytics

We use Google Analytics, loaded through Cloudflare Zaraz, to understand how visitors use this site, such as pages viewed, phone-link taps and completed form submissions; we send only the form type (inquiry or referral), never the names, contact details or messages you enter.
